The original regulation document for the founding of the Central Committee for Overseeing Chabad Matters, the original handwritten document that was apparently handwritten by the secretary of the Rebbe the Rayatz, Rabbi Yechezkel Feigingi. Copies of this document were handed out to Chabad rabbis in Poland in the 1930s.
The document opens with the words "ועד מרכזי לפקח על עניני חב"ד. מתייסד במדינת פולין, במספר של אחד עשר חברים, לבד הרביים החסידיים שנמנים ג"כ לחברי הוועד הזה, מקום מושב הועד בווארשא עיר הבירה" (Central Committee for Overseeing Chabad Matters. Founded in Poland, with 11 members, apart from the Chassidic rabbis also appointed members of the Vaad, the Vaad sits in Warsaw the capital). Then there are dozens of clauses pertaining to the establishment of Torah lessons, Hisva’aduyot, customs of the synagogue, establishing a Kollel Rabbi Meir Baal HaNes, and more.
One interesting clause establishes
"להשתדל בכל האפשריות שכל מנין שמתפללים בו אנ"ש שי’ שיקרא בשם חב"ד, נוסף על השם שיש לו מכבר כמו סטאראשעלער, לאדיער וכדומה ולאשר את זה השם חב"ד מטעה"מ" (to make every possible effort that in any minyan in which members affiliate themselves with the name of Chabad, to add to the name of the minyan in addition to the existing name such as Storoscheller, Ladier, etc…)
