[4], 128 leaves. Stains and much moth damage. The last leaf is mounted to the flyleaf. New simple binding. Fair overall condition.
The Kabbalist Rabbi Yaakov Tzemach of Damascus and Safed was a disciple of Rabbi Chaim Vital and authored many Kabbalistic books.
