"עטרת צבי ספר בית לחם יהודא והוא ביאור חדש מדברי הגאונים הראשונים ואחרונים על שולחן ערוך מטור יורה דעה" by Rabbi Zvi Hirsch ben Rabbi Azriel of Vilna. Printed in Polonne, 1804. [2], 113 leaves. 

There are some variants of this edition that differ in the year of printing and date of approbations. This edition is exceptional in that the date of the approbation by Rabbi Avraham Dov Auerbach Av Beit Din of Polonne and Chmelnik is 1804 whereas the date of the approbation by Rabbi Bezalel Margaliot Av Beit Din of Ostrohe is 1800. In those years it was forbidden to print "for ten years without the printer’s knowledge". In addition, there is difference in the color of the first two leaves, compared to the rest of the book, possibly indicating that they were printed separately. 

No binding. Detached leaves and gatherings. Partly on bluish paper. Stains and some moth. Fair-good overall condition.
