A letter of request from the beit din, a declaration in French with Hebrew translation, a letter handwritten and signed by Rabbi Chaim Yaakov Rotenburg, Av Beit Din of Paris, and a letter handwritten by Rabbi Avraham David Rosenthal, Av Beit din in Jerusalem.

Rabbi Chaim Yaakov Rotenberg (1910-1990) was born in Wadowice, Poland, the son of Rabbi Mordechai Rotenberg (Rabbi in Wadowice and head of the Beit Din in Antwerp). He studied in the Telz, Mir, and Brisk yeshivot. After WWII he was appointed rabbi and Av Beit Din of Antwerp, and in 1964 he immigrated to Paris and was appointed the head of the community association, which he fulfilled until his death. He established and managed educational institutions in Paris for all ages and a large kashrut association. His sister is Mrs. Rachel Sternbuch, who became famous during WWII for immigrating to Switzerland and saving hundreds of Jews from the Nazis.
