SeferHaGoralot, according to the AvneiHaChoshen. Includes guidelines for figuring out the goralot (fortunes), list of questions to ask, and tables for analyzing them. At the top is an addendum pamphlet on halachot of shechita, also handwritten. Sofer script, mostly in Mizrahi handwriting, the tablets and fortunes are in block lettering. 41 pages, 15x19cm. Generally good condition.
